Rates

$180 per hour

Insurance

Dr. Miller is an "in-network" health insurance provider for Blue Cross/Blue Shield (in state), Blue Cross/Blue Shield (out of state). Blue Cross PPO, Blue Care Network, Aetna, ASR/Physician Care Network, MCLAREN Hospital Employee's Health Insurance Plan, COFINITY sponsored health insurance plans, Value Options. Dr. Miller also accepts HSA's. 

If you have a health insurance plan NOT listed, following payment by credit card or check, you may be able to retrieve a portion of the cost for Dr. Miller's services as an "out of network" provider. This depends solely on your health insurance plan and its provisions. While a "superbill" will be provided to you following payment, such arrangements are between you and your healthcare plan and no guarantee of payment by your plan can be provided by Dr. Miller or his office. 

Dr. Miller is NOT a Medicaid provider. 

Depending on your current health insurance provider or employee benefit plan, it is possible for services to be covered in full or in part. Please contact your provider to verify how your plan compensates you for psychological services.

I’d recommend asking these questions to your insurance provider to help determine your benefits:

• Does my health insurance plan include mental health benefits?
• Do I have a deductible? If so, what is it and have I met it yet?
• Does my plan limit how many sessions per calendar year I can have? If so, what is the limit?
• Do I need written approval from my primary care physician in order for services to be covered?

Payment

For deductibles, non-covered services, and self-pay, I accept cash, checks, and most major credit cards as forms of payment.

Cancellation Policy

If you are unable to attend a session, please make sure you cancel at least 24 hours beforehand. Otherwise, you may be charged for the full rate of the session.
